Please take a moment to carefully read the following information If you have a medical condition or specific symptoms, massage therapy may be problematic for you. A referral from your primary health care provider may be required prior to treatment being provided.

 

  • I understand that the treatment I receive is for the basic purpose of relaxation and relief of muscular tension.
  • If at any point during the massage I am uncomfortable or uneasy with the procedures being administered and/or if I experience pain, I understand it is my responsibility to IMMEDIATELY inform the massage therapist, so that the massage can be terminated or the strokes and pressure can be adjusted to a level of comfort.

 

    I further understand that massage therapy is not a substitute for a medical examination, diagnosis, and treatment.
1. Prior to a massage, remove all jewelry. Pull long hair back with a clip.
2. Please provide feedback as to pressure (deeper or lighter) and discuss painful or ticklish areas of your body.
3. Feel free to ask questions about the procedures. The massage therapy provider is well trained, ethical, and professional, and will be happy to make you feel well informed and comfortable. 
4. ALL and  Any illicit or sexually suggestive remarks or advances will not be tolerated and can result in immediate termination of the treatment.
5. Ensure All Pets and non participant parties are not in the same workspace

6. The minimum dimensions for a room are 10′ x 10′. This will give you almost 3 feet on each side of the table
7. Remove any furniture or clutter that would interfere with the massage space.
8. Make sure massage space is clean

9. No Foul, inappropriate, demeaning, threatening or Sexual language towards our therapist will be tolerated.
9. We have the rights to reschedule, refuse or cancel any clients sessions for any of the reasons indicated are not met .

REPLETE CANCELLATION POLICY

Cancellation/Late Policy

We understand that unanticipated events happen occasionally in everyone’s life. In our desire to be effective and fair to all of our clients and out of consideration for therapists’ time, we have adopted the following policies:
  • 24 hour advance notice is required when canceling an appointment. This allows the opportunity for someone else to schedule an appointment.
  •  Appointments made within 24 hours are automatically subject to cancellation fees. Your credit card will be charged for the agreed service(s) 24 hours prior to your appointment. 
  • CANCELLATIONS MADE ONLINE ONLY
  • If you are unable to give your therapist a reasonable time of advance notice you WILL be charged 50% for your missed appointment.
  • No-shows Anyone who either forgets or consciously chooses to forgo their appointment with no contact, will be considered a “no-show”. They will be charged the full amount for their “missed” appointment 
  • Arriving late Appointment times have been arranged specifically for you. If you arrive late your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your therapist will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the “full” session, however we will do our best to satisfy the scheduled time.

COVID-19 PRECAUTIONS

These protocols are in accordance with federal, state, and local guidelines

  • The following guidelines apply to all Massage Therapist delivering Replete Massage services, whether in private homes, workplaces, spas, or hotels:

    • Massage Therapist will wear masks/facial coverings and may or may not wear gloves during treatments as an enhanced safety measure.
    • Each day Replete Massage Therapist confirms he or she displays no symptoms of COVID-19 or other infectious illnesses.
    • Massage Therapist are encouraged to take their temperature daily.
    • Massage Therapist should follow the CDC guidelines on handwashing or using hand sanitizer with at least 60% alcohol content.
    • Massage Therapist may be asked to remove shoes upon entry.
    • Massage Therapist will disinfect equipment before and after every use.

Safety Guidelines for Replete Clients

   As we’ve learned from weeks of global cooperation in the efforts to stop the spread of      COVID-19, efficacy correlates directly to participation. To help keep providers in the  Replete network—and everyone in your community and workplace—safe, we ask Replete clients to follow these protocols:

  • In-home clients should refrain  from booking a treatment if they or a member of their household has a fever, other COVID-19 symptoms or any communicable illness.
  • In-home clients should refrain from booking a treatment if they or any member of their household is under an isolation or quarantine order/directive.
  • In-home clients should use their own sheets on the massage table.
  • All clients—whether in their home, their workplace, a spa, or a hotel room—should wear a face mask.
  • All clients should wash hands thoroughly, prior to and after each treatment.
  • Clients should share any special sanitation or hygiene requests prior to the provider arriving for an appointment. 
  • Clients may offer masks/gloves and no-contact thermometers to Therapist at their discretion.
